Below is the referral process for 1. Healthcare Providers (MD, NP, OBGYN) and 2. Rehabilitation Consultants.
1. Healthcare Providers (MD, NP, OBGYN): How to Refer a Patient
We aim to make the referral process seamless for administrative staff:
- Download the Form: Access the BBDNutrition Dietitian Referral Form on our secure website.
- Fax the Request: Fax the completed form to the fax number listed directly on the form along with a general blood (hematology) panel, ferritin, fasting blood glucose, HbA1C, and a lipid panel (cholesterol), and any other relevant lab results. Please limit the fax to 10 pages or less.
- We Handle the Rest: Once received, our office will contact the patient directly to schedule their intake.
Alternatively, if you prefer for patients to self-refer, you can have them download our one-page BBDNutrition Practice Overview Brochure, or you can print out a copy and provide it to them.

2. Rehabilitation Consultants: How to Obtain a Quote for Services
- Download the BBDNutrition Referral Form and complete it yourself. A doctor’s referral is not required.
- Send the completed BBDNutrition Referral Form along with the client’s most recent complete lab test results, including a general blood (hematology) panel, ferritin, fasting blood glucose, HbA1C, and a lipid panel (cholesterol) to our office.
The client can download their most recent complete lab report by following these instructions: - Once we receive the completed BBDNutrition Referral Form and a copy of the client’s most recent complete lab work, we will reach out to discuss proposed services and estimated costs.
Estimated Costs of Services:
For routine cases, we bill the Assessment Visit as a Standard Clinical Nutrition Appointment ($150); for clients with complex clinical needs, we bill it as a Complex Clinical Nutrition Appointment ($170).
Services for Meal Plan design, the Nutrition Education Session to teach the Meal Plan, follow-up visits, and any requested Provider Reports from the Dietitian will be billed as either a Standard Clinical Nutrition Appointment ($150) or Complex Clinical Nutrition Appointment ($170), depending on the client’s clinical needs.
Standard Clinical Nutrition Appointment: $150 (GST exempt), billed at the standard clinical rate for individualized nutrition care. Appointments are 50 minutes, with 10 minutes for reviewing clinical data, requesting additional lab tests (if needed), and planning next steps.
Complex Clinical Nutrition Appointment: $170 (GST exempt), for complex cases. Appointments are 50 minutes, with 10 minutes for reviewing clinical data, requesting additional lab tests (if needed), and planning next steps.

Q: What clinical documentation should be submitted alongside the referral form?
A: To help optimize the initial clinical assessment, please fax the completed referral form along with recent, relevant laboratory findings, diagnostic reports, or specific dietary orders. To minimize administrative data burdens, please limit the accompanying chart documentation to 10 pages or less.
Q: What is the turnaround time for onboarding a referred patient?
A: Once your clinic faxes the referral form, our office will reach out directly to the patient within 2 to 3 business days to initiate scheduling. If the patient prefers to contact us directly, your staff can provide them with the BBDNutrition Practice Overview Brochure.
Q: Can I refer patients needing Routine Health or Chronic Disease Management, as well as Digestive Health issues?
A: Yes. With 18 years of private practice experience, I can accommodate complex multi-system profiles. This includes patients navigating concurrent metabolic health challenges such as type 2 diabetes (not on insulin) or NAFLD alongside functional GI disorders such as IBS or functional dyspepsia.
