The Comprehensive Dietary Package provides all three services described under the Complete Assessment Package (assessment visit, design of Meal Plan, Nutrition Education Session) PLUS  2 follow-up appointments (30 minutes each).

Step-by-Step instructions for Booking Your Appointment:

    1. Download the Intake and Service Option Form (available under the Services tab)*, and fill it out on your computer* (not by hand, please).
    2. Email the completed pdf form to the email address on the form. 
    3. Pay for your services below via credit card or send an e-transfer and save the credit card charges.

* NOTE: If you don’t already have it, you can download the free Adobe Acrobat Reader here to complete the pdf form on a Windows-based computer. If you are on another type of computer, a phone or a tablet, then you can use Adobe Acrobat online fill and sign tools.

The 2:1 Therapeutic Ketogenic Diet Package is a specialty therapeutic package. 

It provides an assessment visit, design of 2:1 therapeutic ketogenic Meal Plan (which includes the Complex Clinical add on for the time involved in the additional calculations), a specialized Nutrition Education Session for teaching the Meal Plan, food groups, as well as electrolytes and other supplementation, PLUS  a one hour follow up appointment for teaching how to track glucose and ketones and to optimize ratios.
